Job Description

The Senior Nurse in a Senior Care Centre shall also undertake the secondary appointment as a Care Programme Associate.

Key Responsibilities

  • Take charge of the entire clinical nursing practice in a Senior Care Centre
  • Take charge of nursing care for elderly clients; including fall risks and hospitalisation processes.
  • Take charge of medical procedures in an outbreak of infectious diseases.
  • Conduct client assessments during admissions and periodic reviews.
  • Participate in multidisciplinary case discussions.
  • Oversee adequacy of medical supplies and equipment.

Job Holder Requirements and Qualifications

  • Diploma in Nursing – Registered with the Singapore Nursing Board

Key Responsibilities:

Primary Responsibilities and Duties:
  • Maintain a high level of customer service and client/caregiver engagement.
  • Manage the conduct of client assessments during admissions and periodic review and update clients’ Individual Care Plan [ICP].
  • Provide financial counselling on all available schemes of services appropriately for individual client’s admission.
  • Lead and supervise the care team of around 20 to deliver care focused on person-centered client programmes and oversee clients’ safety and care and take an active role in programme design to adjust to the changing client base.
  • Conduct multidisciplinary case discussions with Therapists, Nurse and the care team and ensure that the clients’ care plans are fulfilled for the clients’ well-being.
  • Manage staff recruitment and training needs.
  • Maintain financial accountability by reviewing, forecasting, preparing, and justifying operational and capital budgets and manage client (daycare) intake to fulfill MOH’s requirements.
  • Maintain inventory control of assets and consumables and ensure regular maintenance of equipment.
  • Work collaboratively with the Admin & Finance team on admin/finance issues that need interfacing with centre management and also oversee the day-to-day customer facing operations that the Admin & Finance team is managing.
Secondary Responsibilities and Duties:
  • Establish and oversee a volunteer group to support the conducting of activities in the centre.
  • Understand dementia issues and know how to manage clients who display behavioural problems [associated with onset of dementia].
  • Manage enquiries (in person at centre as well as through other means of communications) with timely follow-up.
  • Participate actively in ad hoc projects as required by Director, Senior Care Division
  • Take charge of following up on clients’ arrears issues to find a resolution where needed.

Job Holder Requirements and Qualifications:

  • Bachelor degree from an accredited University
  • Preference will be given to candidates with a Nursing Degree/Diploma or Degree/Diploma/Specialist Certification in eldercare [or dementia care] from a recognized local or overseas institution
  • Minimum 5 years of relevant working experience in a hospital or eldercare setting and at least 3 years of managerial experience managing a team of staff preferably more than 10 staff
  • Good communication and interpersonal skills and ability to take charge to resolve operational issues expeditiously
  • Possess a compassionate and sincere disposition to care for frail seniors

Key Responsibilities:

  • Support the Centre Supervisor and Executive in the day-to-day execution of activities/related tasks at the Active Ageing Centres.
  • Engage elderly residents in the community and encourage them to participate in activities which the Active Ageing Centre organizes.
  • Organize social activities and outings for the community at the designated Active Ageing Centre.
  • Co-plan and support integrated home-based or offsite programs/activities extended from the Centre and fulfill at least four of the five domains recommended by the regulatory agency, AIC (Physical; Cognitive; Social, Volunteerism, Learning).
  • Engage and support volunteers for programs and activities.
  • Source for suitable locations for the conduct of activities and programs.
  • Deliver a high level of customer service and engagement with seniors and community partners.
  • Attend phone calls to the Centre hotline and walk-ins when the Admin Assistant is away.
  • Support targeted outreach and contact seniors within the Centre’s service cluster at least once annually.
  • Collect and compile data for KPI Submission on a weekly or monthly basis.
  • Attend to any other ad-hoc duties as and when required.

Job Holder Requirements and Qualifications:

  • Minimum “O” Level qualification
  • Preferably with at least 1 year of working experience in an eldercare setting or befriending and buddying services [or volunteering services]
  • Good communication and interpersonal skills
  • Possesses a passionate and sincere disposition to plan activities and outreach to active and frail seniors.
  • Be able to work in the evening and on weekends when necessary/required.

Overview

PCF Sparkle Care is an integrated eldercare facility. The Senior Care Centre provides centre-based community care services for seniors who are physically frail and in need of supervision during the day while their loved ones are at work.

Job Description

  • Plan and run physiotherapy rehab therapy services in the Senior Care Centre [in coordination with the Centre Manager and Occupational Therapist].
  • Conduct assessments for SCC clients during admission and periodic reviews.
