Documents Required

During registration, you will receive our registration form together with a GIRO form and a list of supporting documents to be submitted in a checklist.

  • A non-refundable registration fee is payable upon submission of the Registration Form or upon confirmation of placement for new students.
  • All the listed documents must be submitted and sighted by our preschool in order to process your child’s registration. Forms and relevant photocopied documents will be retained for record-keeping purposes.

Please refer to the list of documents according to the citizenship of your child.

    1. Child’s Singapore Birth Certificate
    2. NRIC of both parents
    3. Health Booklet – your child must complete MMR (Measles, Mumps and Rubella) and DPT (Diphtheria, Pertussis, Tetanus) vaccinations prior to registration, based on National Childhood Immunisation Schedule (Singapore)
    4. CDA Letter/Statement Account* and Bank Account Number for GIRO Application
      *If CDA Account belongs to sibling, please bring along the sibling’s Birth Certificate
    5. Recent passport-sized photo of your child
    6. Legal Guardian’s NRIC, if applicable
    7. Legal documents from lawyers bearing both parents’ signatories confirming legal guardianship of the child, if applicable

    1. Child’s Birth Certificate
    2. Child’s Entry/Re-entry Permit
    3. NRIC of both parents or Entry/Re-entry Permits if they do not possess Singapore NRIC
    4. Vaccination certificate / Health Booklet – your child must complete MMR (Measles, Mumps and Rubella) and DPT (Diphtheria, Pertussis, Tetanus) vaccinations prior to registration
    5. Recent passport-sized photo of your child
    6. Bank Account Number for GIRO Application
    7. Legal Guardian’s NRIC, if applicable
    8. Legal documents from lawyers bearing both parents’ signatories confirming legal guardianship of the child, if applicable

    1. Child’s passport/visit pass/Dependant’s Pass
      *If your child does not hold a valid Long-term Visit Pass (LTVP), Dependent’s Pass (DP), and Immigration Exemption Order (IEO), our preschool will apply for a Student’s Pass (STP) for your child and is subject to assessment and approval by the Immigration and Checkpoints Authority (ICA).

    3. NRIC/ Dependant’s Pass/Passport of both parents
